In third season episode Insatiable, a carnival fortune-telling booth called Sallah the Soothsaying Sultan tells Claudia that she will die before the night is over, and soon thereafter, near-miss accidents start to befall her.  Convinced that she has been cursed by the artifact, Claudia tries to inactivate it before she winds up dead, only to eventually learn that the device doesn't actually tell fortunes, but is in fact more of a hypnotist, creating an overwhelming feeling of dread in anyone who activates it by predicting their impending demise.

Printed thin cardboard fortunes, including the 6 seen onscreen, as well as multiple additional fortune cards, all predicting Claudia's death, for a total of 18 total different fortunes. With multiple extra pieces including individual cards in two color schemes, and strips of uncut cards.
