These Medical Papers were bought from Screenused.com. The COA says these papers were used on screen in the season two episode "Astral Monkey". It also says that these style pages were used as set dressing in the season eight episode "The Lost Picture Show".The pages mention Prue Halliwell, there is one page with identification summary, admission summary and discarge summary, one for physical examination, one for authorization to receive or release medical information and the last page has Prue's temperature, pulse and respiratory information on it.
