This name patch was part of the uniform style worn by all Seaquest's crew during all 3 season of the show. The patch lists the character's name and the vessel the character is serving on, in this case the character is M. Crocker who is serving on the vessel DSV 4600. This name patches are located on the on the right hand chest of the uniform and are attached via a Velcro strip on the back of the patch to the uniform.

This particular name patch is specific to one episode only, which is the first season pilot episode titled "To Be Or Not To Be".
