Produced as a three-part television miniseries by Walt Disney in 1963. It starred Patrick McGoohan of Danger Man/Secret Agent and The Prisoner fame. This is a stylised and more earthier replica version on the hero mask. Using natural materials like sackcloth to give the whole look a much more menacing quality. It was made by a US propmaster and is, as far as I know, a one-off.
